Human Services Coord (A/B)

The Department of Mental Health, as the State Mental Health Authority, assures and provides access to services and supports to meet the mental health needs of individuals of all ages, enabling them to live, work and participate in their communities. The Department establishes standards to ensure effective and culturally competent care to promote recovery. The Department sets policy, promotes self-determination, protects human rights and supports mental health training and research. This critical mission is accomplished by working in partnership with other state agencies, individuals, families, providers and communities.
Responsible for discharge planning and subsequent service coordination for hospitalized clients at psychiatric units, and at Transitional/Respite programs.
Maintains an assigned caseload.
May provide information, referral and short-term linkage services to those who are not DMH eligible, or those eligible on a waiting list for services.
Develops and writes the Comprehensive Assessment and Individual Service Plan, in accordance with CMR 104.29; submits to supervisor for review and sends copies to vendors involved in client's care and support to ensure that needs are addressed in a comprehensive and appropriate manner. Develops Individual Risk Management plans for clients, as needed.
Links clients with appropriate agencies and organizations to meet client's needs identified through the ISP process and ensures that necessary services are provided in the least restrictive environment.
Provides input regarding assessment of client's strengths and needs to supervisor or other clinical service staff in order to facilitate appropriate client care across all Area services.
Develops a helping and supportive relationship with clients, their families and significant others in order to encourage clients to function as independently as possible.
Travels to meet with clients and family members outside the office in various settings such as homes, hospitals, programs and other accessible community settings.
Forms working relationships with service providers and acts as the contact to resolve issues related to the provision of services as described in the client's Individual Service Plan in order to maintain a single point of accountability for all case managed clients.
Maintains a courteous and helpful demeanor in dealing with the public.
Prepares consultation materials for presentations and follows up on relevant recommendations.
Advocates for client's rights with all community providers, agencies, families, court systems, employers and significant others in order to ensure that client's rights are recognized and respected.
Assist with clinical evaluations of Adult Request for Service Applications submitted to DMH Western Area office. Assess applications in accordance with clinical criteria specified in DMH regulations and guidelines. Apply all criteria related to Needs and Means assessment to help make full determination when necessary.
Attends case management staff meetings and relevant trainings sessions, seminars, conferences and academic courses in order to maintain and improve current knowledge of community treatment of the long term, seriously mentally ill, or seriously emotionally disturbed.
Attends ongoing and regular weekly supervision meetings, e.g. clinical reviews, staff meetings, to ensure a high quality of service delivery to clients.
Communicates relevant information to FTT regarding changes in client's forensic status.
Reviews program treatment plans (IAPs) to insure consistency with DMH service plan.
Attends Individual Action Plan meetings for clients in DMH-funded programs as needed.
Provides or insures that DMH contracted program staff provide life management supports as needed such as assisting clients with placements, monitoring appointments with medical/mental health providers; arranges transportation when necessary, finding appropriate housing for clients, coordinating visits to prospective residences, addressing money management and representative payee needs, and assisting clients with applications for income (SSI, SSDI, DTA, EDC, Financial Assistance) including accompanying the client to the agency in order to ensure that the application/approval process is completed as soon as possible to facilitate the client's transition into community life.
Refers, assists in the application of, monitors and assures client's receipt of medical insurance entitlements (Medicaid/Medicare) to ensure broadest access to medical care. May accompany the client to the respective agency.
Maintains all required documentation and records including, but not limited to, Comprehensive Assessments, Individual Service Plans, Annual Reviews, MHIS data, DON, progress notes, data requests, reports of team meetings, correspondence and other data so to meet pertinent accreditation and regulatory standards within given timeframes.
Identifies gaps in services and other system problems via participation in the Quality Improvement and Utilization processes for case management in order to maintain professional standards of client care within the Case Management Program.

The DMH Western Mass Area promotes services that are clinically appropriate, least restrictive and delivered close to home. Services are designed to be strength-based, recovery-oriented and culturally/linguistically competent. Given the population served, bilingual or multilingual candidates are encouraged to apply. Strong computer skills are needed.

MINIMUM ENTRANCE REQUIREMENTS:
REQUIRED WORK EXPERIENCE: At least three years of full-time, or equivalent part-time, professional experience in human services work or social work. Based on assignment, one year of professional work experience must have been in one of the following specialty areas. Refer to the individual position description for specialized work experience required.
Mental Health Case Manager and Forensic Transition Team Coordinator assignments only: The year of professional work experience must have included the clinical care and treatment of psychiatric or emotionally disturbed individuals.
Department of Developmental Services assignments only: The year of professional work experience must have involved working with individuals with intellectual or other developmental disabilities.
SUBSTITUTIONS:
A Bachelor's or higher degree in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, or physical education may be substituted for two years of the required experience on the basis of two years of education for one year of experience.
Mental Health Case Manager and Forensic Transition Team Coordinator assignments only: Degree substitutions must be in social work, psychology, sociology, counseling, counseling education, rehabilitation, rehabilitation counseling, human services, education of the physically or emotionally handicapped, education of the multiple handicapped, or education of the learning disabled.
Department of Developmental Services assignments only:
A current and valid Massachusetts license or registration as a physical therapist, speech-language pathologist, occupational therapist, audiologist, registered nurse, dietitian, or social worker may be substituted for two years of the required experience.
A current and valid Massachusetts license or registration as a physical therapy assistant or occupational therapy assistant may be substituted for one year of the required experience.
One year of education equals 30 semester hours. Education toward a degree will be prorated on the basis of the proportion of the requirements actually completed.
No substitutions will be permitted for the required year of professional experience.
REQUIRED EDUCATION AND LICENSES:
One of the following is required for Qualified Developmental Services Professional assignments in the Department of Developmental Services only:
A current and valid Massachusetts license or registration as a physical therapist, speech-language pathologist, occupational therapist, audiologist, registered nurse, dietitian, or social worker, OR
A Bachelor's or higher degree in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, or physical education.
Based on assignment, travel may be required. Those employees who elect to use a motor vehicle for travel must have a current and valid Massachusetts Class D Motor Vehicle Operator's license or the equivalent from another state.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
